To ensure the best possible care, it is essential that your medical chart reflects the most current and accurate information about your health. Please follow these steps to become a valued patient and get you ready to make your first appointment.

1. Register for Patient Portal
   Send us a request for the link to self register for the patient portal by filling out this form, or call the office at (719) 275-1037 to request a link be emailed to you.
   
   
2. Complete Registration
   Click on the link provided in the email you received and carefully follow the instructions to register your patient portal.
   
   
3. Update Medical Record
   Please complete your new patient paperwork by clicking the "review medical record" tile. Click through each subtopic to edit/add information to your medical chart.
   
   
4. Schedule Appointment
   Either request an appointment through the portal, call the office to schedule your appointment, or wait until one of our staff members reaches out to get you scheduled.
   
   
5. Pre-Appointment Check-in
   At least one day prior to your appointment, log back in to your portal and click the "check in" tile. Carefully answer the questions until the check in process is complete.
